Volunteer Forms
  • ALL Volunteers who wish to help in the Preschool through High School are required to submit a Volunteer Background Check Form (PDF) each year and submit to the school office.
  • School Vehicle Driver Form (PDF)
    Anyone who will be helping to drive the vans for field trips must submit a completed School Vehicle Driver Form, along with a copy of your valid drivers license, to the main office at least 2 days prior to the drive.

Medication Authorization

➜ Medication Authorization Form (fillable PDF)

As mandated by state law, we cannot administer students’ prescription or over-the-counter medications (including cough drops and vitamins) unless we have an Authorization for Medication form on file in the office. The Authorization for Medication form must be completed and signed by a licensed health professional and parent. A new Authorization for Medication form is required each school year. Oral medication, topical medication, eye drops or ear drops, and life-saving medications can be administered. Forms are available at the school office or from the student’s doctor’s office. Medicine must be in its original container, labeled with student’s name and not expired. Prescription medications must have the student’s prescription label attached. Students are not allowed to carry medication of any kind without preauthorization from office and administration. If your child has a life-threatening allergy or other serious condition, please let the office know as there may be additional emergency forms needed.

Hot Lunch

Emergency Kits

All students are required to have an emergency kit at school. You may create your own kit with like items or go to Emergency Kit Order Form 2025-2026 (PDF) [NEEDS LINK] to order a kit and return to the front office by August 28 (Back to School BBQ).

5th-8th Grade Athletics

  • Priority registration through Thurs., Aug. 28, 2025
  • Open registration to include non-NCA families: Fri, Aug. 30 – Wed., Sept. 3 
  • Fall athletic forms must be turned in by Sept. 3, 2025
  • Fall athletic parent meeting: Tues., Sept. 10, 2025, 5:00-5:30 pm, in the NCA gym
  • All sports are capping at 24 athletes

Extended Care

  • Register for before/after school care starting Sept. 3 (first day of school).
  • Cost for before/after school care is $6 per hour from 6:30-7:45 AM and 3:00-5:30 PM.
  • Registration is required.
  • Families who utilize our before/after school Extended Care program will need an access card for drop off & pick up. Please fill out the Security Card Request (Google form).

Theater Registration for the 2025 Fall Production of ______

  • 2nd-8th grades
  • For more info regarding cost, auditions, rehearsals, and performance dates, go to Register for Theater (Google form).
  • Registration is required and due Sept. 10.

Music Groups

[UPDATE info]

This year we will be offering Rock Band on Tuesdays and Orchestra on Thursdays, from 3-4:30 pm, for grades 4-8. Any students in grades 2-3 who would like to join will be required to audition.

Groups will be offered in 3-month sessions. First session is Sept. 9-Nov. 26 (no lessons on non-school days or half-days).

Cost is $85 per month ($255 per session), invoiced via your FACTS account.
(International students, please bring payment to the front office.)

Student Parking

  • All students who will be driving a vehicle to school will need to apply for a student parking permit and submit to the NCHS Main office. Students cannot park on campus without a parking permit.

NCHS & Beyond

Go to the ➜ NCHS and Beyond Page for grade specific information or to download a Student Community Service Ledger and Community Service Criteria for students who would like to track volunteer hours over the summer. This is separate from the Family Stewardship Program.

SAT & PSAT Testing

➜ High School SAT & PSAT

College Board is now all-digital for the SAT and PSAT tests. If your sophomore, junior, or senior is considering taking one of these tests, the deadline to register is Thurs., Sept. 4, 2025. Any student who does not register by this date will not be able to take the test.

NCHS Café

[INFO -DATE?] The NCHS Café will begin serving lunch Sept. 5 and will continue throughout the school year on Wednesday and Thursday of each week. Our ASB Leadership students serve Costco Pizza for purchase every Friday. Muffins are available daily. Muffins and pizza slices are $2 each.
